Post-Hospital Care & Rehabilitation Unit (Skilled Nursing & Rehabilitation)
Asbury Court and Asbury Gardens provide rehabilitation care to injured, disabled persons in need of physical, occupational and speech therapy. Working with Select Rehabilitation – an industry leader in providing licensed therapy services – our highly skilled interdisciplinary healthcare team provides each patient with an individualized plan of care, developing the most appropriate treatment to provide them with the best possible outcome and a return to independence prior to discharge.

Activities
Our exceptional, interdisciplinary healthcare team is committed to providing a safe, comfortable environment that residents can personalize and will love to call home. We provide private apartments with services based on an individualized, confidential assessment.
A service plan is developed for each resident by a campus team with the resident’s participation, interests, preferences and needs in mind. Our goal is to find the right level of assistance that offers a variety of choices that will allow residents to socialize and participate in the activities that they prefer along with services that will promote their independence. Our activity program offers shopping and community outings, live entertainment, exercise and group activities that allow residents to socialize and participate as much or as little as they desire.
Independent Living
Asbury Court’s Independent Living allows seniors to enjoy an active community focusing on health + wellness. Our Independent Living allows you the freedom you want with the service you deserve. We offer engaging Life Enrichment Programs, 3 meals a day served restaurant style as well as laundry and housekeeping services. You will have all the comforts of home while living life to its fullest.

Supportive Living
Asbury Court is part of the State of Illinois Supportive Living Program. The program is designed to promote the independence, dignity, respect and wellbeing of residents in a cost effective manner.
Supportive Living services are available at these Asbury locations:
Medicaid
Depending on an individual’s financial and health status, Medicaid will reimburse Asbury Senior Health and Wellness Communities for a qualifying resident’s cost of service in the Supportive Living Program (SLP). Residents of this program participate in their personal service plan, they control their own lives.
Personal and Medical Services
Residents of Asbury’s Supportive Living program are offered a wide array of support services and amenities designed to promote their personal comfort and peace of mind.

Supportive Living Memory Care
Our Supportive Living Memory Support Program provides person-centered, activity-based care for seniors that exhibit symptoms related to internal pathological changes in the brain. Those symptoms must affect intellectual and social abilities severely enough to interfere with daily functioning, in that it is unsafe for them to reside alone in accordance with the Illinois Supportive Living Dementia Care guidelines. We utilize the Alzheimer’s Association Life Services Network, Activity and Social Service consultants and a variety of other long-term care resources to assist us with interventions and delivering quality memory care programming.
